Clay County is actively enrolling partners into our continuum of care program called Community Services. There are several goals for the Community Services platform.

The first goal is to improve the quality of life of Clay County residents by connecting them with the providers of the services for which they are in need. Community Services makes sending and receiving referrals very efficient and easy to track, inspiring collaboration between organizations and maximizing benefits to children and families. This program allows all partner organizations to send and receive referrals to one another within seconds.

The Community Services platform's second goal is to bolster partner organizations' capabilities. Community Services can be more than just a referral software for interested parties. It is built to be a comprehensive internal case management program. The program has dozens of pre-built reports and a custom reporting option, allowing providers to run real-time data reports as needed.

The final goal of Community Services is to identify service areas in which there are not currently sufficient providers to meet the community's needs so that the County can help fill in those gaps.

Community Services is very customizable, making it simple to identify and display the information and content relevant to each specific provider or user’s preferences. Counts Reports allow a snapshot of referrals and clients on the home page.

Community Services is a big part of the County’s Strategic and Community Health Improvement Plan. The data collected from this platform will help inform future decision-making to meet our community's needs better.
