Many “high utilization patients” within the county frequently call 911 EMS response services for reasons other available resources may best manage. These calls often involve falls, lift assists, or other reoccurring incidents.
Our Community Paramedicine program personnel meet with these individuals in their homes, provide a cost-free assessment, and then, if the patient desires, assist the patient with aligning the most appropriate resources for their care with improving independence and quality of life at home.
There are many cost-free or volunteer resources the Community Paramedicine program can connect patients with. An ever-growing robust list of volunteer organizations, programs to assist individuals, mental health care opportunities, no-cost medical care, and much more allow the vulnerable population to receive the needed help. Personnel will track patient progress with frequent follow-up visits until the issues have been resolved or our assistance is no longer needed.
